RN Care Manager Outpatient in Dingmans Ferry, Pennsylvania

Poplar Care Network is seeking a Full-time Registered Nurse with a background in outpatient and ambulatory settings and a passion for preventative and lifestyle medicine, to join our growing team.

We are seeking a motivated and passionate RN Care Manager to join the Poplar team who can work on-site at Dingmans Medical Center, one of the practices in our network, to support their
Medicare population, while also providing remote telephonic care management support to other practices in the network. The goal of the RN Care Manager is to assist Medicare patients with developing healthy lifestyle related goals, creating a plan to achieve those goals, all while providing support along the way to assist in managing real life issues that can prevent patients from achieving those goals.

For about 32 hours per week, you will manage a panel of patients at the Dingmans Medical
Center, and use your nursing, coaching, and wellness skills to help patients stay safe and healthy and free of the need to go to the hospital. You may do some home visits where strategic. For the remainder of the week, you will work remotely and service patients in other cities in select Poplar Care Network practices.

Initially, most of the work will be in person at the Dingmans Medical Center during the day shift,
Monday-Friday, with an option for some flexible scheduling. This position would primarily be in-person, with an option to work remotely when working virtually with other practices in the
Poplar Care Network.

While working at Dingmans Medical Center, you will have a dedicated work space while being employed by Poplar Care Network and helping to implement different services we offer to our network of providers.

Responsibilities:

To support work-life balance and fulfillment, this position will rotate between onsite and remote work and provides opportunities for advancement. RN will:

• Support all aspects of Medicare patient care, with a focus on patient triage and patient education

• Develop trusting relationship with patients by providing support and advocacy to help achieve health goals

• Coording transitions of care to/from acute inpatient, skilled nursing and other settings
• Assist patients in identifying and accessing community resources
• Preform wellness checks in homes, participate in remote monitoring program education
• Facilitate communication between all identified parties by the patient involved in care

(e.g., family members, care givers, medical providers, community-based organizations).

• Stand up Advance Care Planning Program for Dingman’s Medical Center
• Perform call backs to Medicare patients, review lab results, schedule referrals
• Perform home visits to Medicare patients of Dingman’s Medical Center
• Assist MD in shared medical appointments
• Act as a point of contact for the Poplar Care Network, to communicate information with the Dingman’s Medical Center practice
• Champion Poplar services at practice
• Any other related duties at managers discretion

Qualifications:

• Must have active and unrestricted licensure as a Registered Nurse (RN) meeting all state certification and licensing requirements in the state of Pennsylvania

• Must meet state and certification requirements to obtain an RN compact license or license in other states

• Minimum of two (2) years of nursing experience related to management of chronic care management and chronic health conditions in a Primary Care setting

• Verifiable good driving record and reliable transportation
• An understanding of preventive and Lifestyle Medicine and passion for this approach to health
• Knowledge of serious medical conditions (such as COPD, CHF, cancer, dementia, diabetes, ESRD, etc.) and their progression
• Excellent phone/video communication
• Strong skills in coaching, “teach it back to me”, and Motivational Interviewing preferred

We offer a health reimbursement account, paid holidays, and 4 weeks paid leave after the first 3 months of employment. We offer 3% matching retirement contributions after the first year.
