Position Description:
The Supports Intensity Scale (SIS™) Evaluator is responsible for scheduling and completing administration of the SIS, to include scoring the assessment and preparing a summary report. In addition, the SIS™ Evaluator may conduct other assessments/evaluations/personal outcome measures as requested.
Responsibilities:
- Assessment/Evaluation and Collaboration - Schedule and administer the Supports Intensity Scale (SIS) as assigned. SIS Evaluator may review clinical evaluations, as needed, to assess current cognitive and adaptive functioning, gather relevant facts of psychosocial history and current status, review diagnoses of the most current DSM, and provide referral resources and recommendations as appropriate.
- Prepare a written report of the results of assessment, including conclusions/recommendations as appropriate, in a timely manner. Assist treatment team with understanding the overall results of the SIS, as well as, incorporating the results of the SIS into the Individual Support Plan. Assist with provision of education/training as needed to promote positive outcomes of SIS initiative. Ensure responsiveness to feedback/questions/concerns from individuals, families and providers related to the SIS. Collaborate with other members of the SIS team to ensure consistency between interviewers and over time.
- Assist as needed with development of operational/training materials related to Supports Intensity Scale. Participate in Inter-rater Reliability for SIS Evaluators on a schedule as determined by AAIDD.
Education/Experience: Graduation from a four-year college or university with a bachelor’s degree in education, psychology, nursing, child development, counseling, social work, child and family studies and two years of professional experience working with the population served or graduation from a four-year college or university with a Bachelor's degree and four years of professional experience working with the population served. Must meet the requirements of a Qualified Professional as defined in North Carolina General Statute.
Additional Training Preferred:
Certification in SIS Assessments preferred. Selected applicant will be required to train and pass certification to become a SIS Evaluator. Proficiency in Microsoft Office products (such as Word, Excel, and Outlook) is required
