Supervision includes reminding and/or observing residents while they perform activities. Assistance includes either physical assistance or standby supervision.

POSITION SPECIFICATIONS:

QUALIFICATIONS:

Minimum Qualifications: Must demonstrate competency in the following areas:
- Vital signs.
- ADLs/IADLs.
- Medication supervision (within the scope of the state regulations).
- Read, write, speak and understand the English language.
- Freedom from illegal use of drugs.
- Freedom from use and effects of the use of drugs and alcohol in the workplace.
- Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a healthcare-related setting are ineligible for employment in the position.
- Demonstrates effective time management skills.
Education and/or Experience:
- High school diploma or general education degree (GED), or three-six months related experience and/or training or equivalent combination of education and experience desired.
Certificates, Licenses, Registrations:
- Must have valid, unrestricted certification in the state (CNA desired) and maintain CEUs.
- CPR/First Aid Certification desired (unless required by the state).
LANGUAGE SKILLS:
- Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, staff members, representatives of community and government agencies.
MATHEMATICAL SKILLS:
- Ability to add and subtract three-digit numbers and do simple multiplication and division.
REASONING ABILITY:
- Ability to solve practical problems and deal with a variety of concrete variables.
- Ability to interpret a variety of instructions furnished in written oral, diagram, or schedule form.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each key function satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the key functions.
Resident Care:
- Gives report to the Nurse or designated charge person at the end of the shift.
- Reports any changes to the Nurse or designated charge person immediately for determination of contacting the on-call Nurse.
- Reminding residents to take medication, opening containers for residents, opening prepackaged medication for residents, reading the medication label to residents, observing residents while they take medication, checking the self-administered dosage against the label of the container, and reassuring residents that they have obtained and are taking the dosage as prescribed.
- Supervise, assist or provide showers and baths.
- Supervise, assist residents to dress/undress.
- Assure the cleanliness and dignity of each resident.
- Supervise or assist with dining services, meal assistance/adaptive equipment, and snacks.
- Supervise, assist or provide grooming activities, (oral hygiene, hair care/shampoo shaving).
- Supervise, assist or toilet residents and assist or provide with incontinence supplies/care when appropriate.
- Escort residents to meals and/or activities.
- Supervise, assist or transfer residents when necessary.
- Report unusual conditions, observations, and pertinent information to the nurse.
- Comply with resident rights, abuse reporting, and confidentiality policies.
- Foster and maintains effective working relationships between peers, professional staff, and other personnel within the community.
- Supervise or assist with IADL’s meal preparation, light housekeeping, laundering of personal clothing, and telephoning.
- Supervise or assist with the use of transportation.
- Supervise or assist with activity programs.
- Promptly respond to resident emergency call lights.
- Comply with all documentation policies/procedures.
- Confidentiality of all residents is respected at all times (i.e., when answering the telephone and/or speaking to co-workers).
- Approach all residents in a kind, gentle and friendly manner. Respect for the resident’s dignity and privacy is consistently provided. Maintain the confidentiality of all resident information.
Guest Relations:
- Demonstrate a courteous, polite, and friendly attitude with residents, families, and visitors.
- Respond to the needs of residents, families, and visitors by providing immediate assistance.
- Assist in maintaining contact with residents who are not involved in activities due to illness or injury.
- Function as a member of the team, assisting and supporting co-workers as needed.
Safety:
- Use proper body mechanics when bending or lifting.
- Maintain knowledge of and comply with the community’s Infection Prevention policies and procedures.
- Know the community’s disaster and fire safety policies and procedures, including the location and correct use of the fire alarm system.
- Follow the designated plan of action in the event of a fire or other emergency.
Performance Requirements:
- Perform all duties according to policy in a professional manner.
- Comply with community attendance policies.
- Comply with community dress code policies.
- Attend scheduled staff meetings and/or in-service programs.
- Performs other duties as assigned.
Resident Rights:
- Promotes and protects residents’ rights, assists Residents to make informed decisions, treats residents with dignity and respect, and protects residents’ personal belongings.
INTERPERSONAL SKILLS:
- Demonstrates active listening techniques, gains support through effective relationships, treats others with dignity and respect, and seeks feedback.
CONTINUING EDUCATION:
- Attends in-service and education programs and attends continuing education required for maintenance of professional certification or licensure.
PHYSICAL DEMANDS:
- Work beyond normal business hours, weekends, holidays, and severe weather situations when necessary.

- While performing the duties of this job, the employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, talk or hear.
- The employee is occasionally required to sit, climb or balance and stoop or kneel.
- The employee will assist to lift and/or transfer residents weighing between 100 and 250 pounds.
- Specific vision abilities required by this job include close vision, distance vision, and peripheral vision.

Exposure Risk:
- Is subject to potential exposure to infectious waste, disease, conditions, etc., including but not limited to, TB, HIV, and Hepatitis B.
