The RN Case Manager will be based out of our Portsmouth, NH office and will service the Greater Seacoast territory. The Nurse will hold a valid Nursing License (RN) in the State of New Hampshire and have a minimum of two years of acute care experience. The RN Case Manager is responsible for managing a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community.

Pay: $36-$40/hour

Job Requirements of the Home Health Registered Nurse Case Manager:

• Valid Nursing License in the State of New Hampshire
• 2+ years of experience in skilled care preferred
• 1+ year of experience in nursing supervision preferred
• Recent wound care experience preferred but is not required
• Current CPR Certification required (We offer monthly renewal classes paid for by Interim HealthCare)
• Reliable transportation
• Home care experience preferred

• A Sign on Bonus of $5,000 is offered to RN Case Managers that have two years of current nursing experience working at least 30 hours per week. After 90 days of employment 20% of the sign on bonus will be paid out. The remaining sign on bonus will be paid after 6 months of employment.
