Seeking to enter a contract with a qualified Physical Therapist while we begin our Medicare skilled division!
We serve clients in Mason, Manistee and Oceana counties.
Requirements
- Physical Therapy license in the State of Michigan (required)
- 1 year of home health care (preferred)
- Ability to communicate effectively and work as a part of the team
- Ability to consistently connect with clients in a one-on-one environment
- Responsible, compassionate and caring demeanor
Job Responsibilities:
- Provide physician ordered Physical Therapy, intermittently, to patients in their homes.
- Supervise PTA’s (PT’s only)
- Work with other health care professionals within the interdisciplinary team regarding patient care and treatment.
- Consult with physicians and staff regarding changes in status and overall progress.
- Teach patient’s and family/caregivers home exercise programs that will ensure patient safety as well as rebuild strength and independence.
- Perform initial evaluations, 30-day reassessments and recertifications/discharges as indicated. (PT's only)
- (PTA's) Perform weekly visits as indicated by patient's PCP.

St. John’s Health Care P.C. has been in business for over 20 years and is a CHAP accredited Private Duty Nursing Agency that takes care of clients in their own homes while assisting with activities of daily living. We are opening a skilled division that will provide intermittent visits to homebound patients in Mason, Manistee and Oceana Counties!
