The Community Care Advocate (LPN) will enhance the services provided by the clinical team in the medical center by being a liaison to patients in the community. By providing visits to patients’ homes and carrying out instructions and plans of care provided by the Primary Care Physician (PCP), the Community Care Advocate will be a quintessential member of the Dedicated care team.

*This role requires daily travel both to the healthcare office on West Lehigh Avenue and in-home visits to patients. It is full time, 40 hours/week but does require working Saturdays with one day off in the middle of the week.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

  • Under the guidance of the PCP, provides education to patients, gathers health updates, and facilitates telehealth visits with the PCP.
  • Supports clinical team and patients with completion of clinical quality measures and closure of clinical gaps.
  • Communicates with PCP and clinical team via telephone, instant messaging, and/or electronic medical records.
  • Develops relationships with patients and caregivers.
  • Facilitates patient care by reinforcing the availability of both in-house and outside appointments.
  • Assesses patient need for transportation to and from appointments and coordinates with center to schedule.
  • Maintains a strong working relationship with PCPs and other members of the care team.
  • Links patient and family with community resources when necessary.
  • Attends meetings and reports on patients as assigned.
  • Maintains a database of patient visits, interventions, and progress and reviews these with PCP weekly and as needed
  • Performs other duties as assigned and modified at manager’s discretion.

EDUCATION AND EXPERIENCE CRITERIA:

  • High school diploma or equivalent education (GED) required
  • LPN (PA) required
  • A minimum of two (2) years’ working in a healthcare setting with patient-facing responsibilities working directly with primary care physicians in an outpatient setting
  • Valid Driver's License with a clean driving record

Job Type: Full-time

Pay: $24.00 - $25.00 per hour
