Licensed Practical Nurse
Job description
Duties:
-Develop strong relationships with patients in order to assist them with their care
-Effectively and professionally communicate with clients and providers throughout the process of accessing care with clients, families, providers, accounts, and others within the healthcare team throughout the process of client's care
- Occasional education or support for caregiving staff
-Creatively troubleshoot roadblocks
-Connect patients with community resources as needed
Requirements:
-Be a current LPN
- Critical thinking skills especially with coordination of individuals specific needs in the home
- Clinical knowledge and communication with partners to help support client's independence and health
- Strong work ethic, teamwork, flexibility
Job Type: Full-time
Pay: $18.00 - $22.00 per hour
