Job Description
Position Summary:
Primary responsibilities include identifying specific needs of Westmed patients admitted to hospitals, developing a plan of care based upon these needs, educating patients and families on warning signs, reconciling, and reviewing medications, reviewing discharge plans, and assisting in appropriate resource allocation. Will work in collaboration with the Westmed Hospitalist team, Transitions of Care Coordinator, Social Workers, and Care Managers.
Essential Functions and Job Responsibilities:
- Work on-site at hospital, identifying specific needs of Westmed patients and informing/educating patients, caregivers and their families of clinical and social services available based upon these needs.
- Acts as a liaison between WMG Hospitalist, hospital Care Management team, PCP, specialists, and embedded Westmed Social Workers and Care Managers.
- Shares medical and psychosocial information between relevant parties as part of the patient review, goals of care, and discharge planning.
- Reviews and utilizes daily WMG Hospitalist signouts to maintain current knowledge of patient status.
- Communicates information pertinent to patient’s medical and social well-being with Transitions in Care Coordinators (TCC), WMG Providers, and/or specialist prior to discharge.
- Develops care plans for complex cases in collaboration with patient/caregiver/provider, Hospitalist team, TCC, WMG Providers and Care Coordination team.
- Provides educational support to patient/family/caregiver prior to discharge, including identification of symptoms specific to patient’s condition which may increase risk for readmission.
- Works directly with each patient from first day of admission to advocate for transfer to partner SNFs when appropriate.
- Communicates any social, environmental, and/or functional needs of hospitalized patients with Transitions in Care Coordinators and WMG Care Coordination team to ensure seamless follow-up.
- Participates in weekly interdisciplinary rounds of hospitalized patients with WMG Hospitalists and WMG Care Coordination team.
- Acts as liasion to identify ACO beneficiary candidates for three (3) day SNF waiver; ensures eligibility criteria is met and reviews options with patient/family.
- Solves complex problems and/or conducts moderately complex analyses impacting transitions/readmissions.
- Discusses appropriate community resources, such as palliative care, hospice, home care and long term care programs with patients/caregivers and families.
- Works with minimal guidance, seeks guidance and assistance from TCC only when needed.
- Provides explanations and information to others on difficult issues.
- Assesses the effectiveness and quality of services provided related to clinical, functional, and financial impact and outcomes.
- Takes in-bound calls and places out-bound calls as dictated by consumer and business needs.
- Adheres to ethical, legal/regulatory and accreditation standards.
- Other duties as assigned.
General Qualification Standard & Job Requirements:
Education/Certifications/Licenses:
- Bachelor of Science degree required
- Minimum of five years clinical nursing experience
- This role requires a valid, NY Registered Professional Nurse license
• Must be willing to obtain CT Registered Professional Nurse licensure • Care management experience a plus.
Performance Standards for Specialty Knowledge, Skills & Abilities:
- Astute triage skills required.
- Care management experience desired.
- Transitional coaching experience a plus.
- Home care, hospital or SNF experience highly desirable.
- Knowledge of Medicaid, Medicare, and government/CMS regulations preferred.
- Self-directed worker.
- Bilingual preferred.
- Must have computer skills and technical aptitude.
- Must be willing to work in hospital setting full time

Pay Range $39/hr to $47/hr
The provided compensation range is based on industry standards and salary determinations will be made based on numerous factors including but not limited to years of experience and location of position.
