At Fortitude Hospice, we welcome candidates who share our purpose and commitment to providing compassionate and quality end-of-life care to our patients and their families. We are looking for a part-time or full-time, or on-call Registered Nurses experienced in hospice care. The general responsibilities include but not limited to:

• Completing ongoing comprehensive nursing assessments of patients and their families to determine and fulfill their needs.
• Managing the services provided to patients and their families in all settings to ensure suitable and quality care.
• Executing the approved plan of care including monitoring and responding to changes as directed by physician.
• Initiating the plan of care of assigned patients including follow-up nursing visits, continuous assessment for signs and symptoms of pain and discomfort, and establishing the appropriate plan of care.
• Creating notes and documentation including electronic submission as necessary for compliance and continuous monitoring of care.
• Communicating and working with Case Manager on assigned caseloads to ensure integrated service and open communication.

Our ideal candidate possesses at least one-year of progressive hospice nursing care experience with a valid Registered Nurse state license, drivers’ license, auto insurance, reliable transportation, and the ability to travel to assigned patients’ locations. To ensure the qualifications of team members, candidates must be willing to undergo a pre-employment background check, physical examination, and TB test.

• Nursing: 1 year (Preferred)

License/Certification:

• BLS Certification (Preferred)
