POSITION SUMMARY:
The Home Health Occupational Therapist will, supervise, evaluate, and provide occupational therapy services according to accepted standards of treatment to patients in the home, as prescribed by the attending physician
ESSENTIAL FUNCTIONS: To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation; including, but not limited to:

  • Conduct Occupational Therapy assessment/evaluation to determine the patient’s level of function in areas including but not limited to strength, endurance, and ability to perform activities of daily living
  • Make recommendation and demonstrate follow-up on assessment findings appropriately
  • Attend interdisciplinary team meetings
  • Establish plan of care with functional and measurable goals within specific time periods
  • Participate with staff, patient in prescribed therapeutic, self-care and creative activities that are directed toward improving independence as well as physical and mental functioning
  • Guide the patient with established household management routines
  • Instruct patients and families in the use of prosthetic, orthotic and assistive devises and the total occupational therapy program
  • Educate other health team members in the care of the patient regarding occupational therapy
  • Supervision of COTA’s, as indicated and may assist with Performance Evaluations
  • Collaborate with physicians, patient/patient representative(s), nurses, therapists, social workers and other members of the care team to develop and implement a patient individualized plan of care/treatment plan

Required Skills:

  • Work requires advance knowledge of the Homecare Industry, Medicare Regulations, CMS, and other State/Federal regulatory changes

Required Work Experience:

  • One (1) year experience as an Occupational Therapist
  • Home Health experience preferred

Licensing/Certification/Education Requirements:

  • Graduate of an occupational therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association or its equivalent.
  • Current Occupational Therapist (OT) License
  • Current CPR Certification

Other Requirements:

  • Position may require unscheduled overtime, week-end work
  • Must be willing and able to work flexible hours
  • Must have reliable transportation
  • Knowledgeable of DME
  • Travel required

Physical Requirements:

  • Frequent lifting of objects up to 40 pounds
  • Work is performed in a standard home health environment

Supervisory Responsibilities:

  • May supervise support staff
