Community Habilitation Provider
Job description
Self-directing individuals who live in the Sidney area are seeking an organized, motivated and patient person to assist them in the following areas: maintain meaningful connections in the community, health/hygiene skills, daily living skills such as teaching money management, cleaning/organizing, healthy choices, cooking etc. Must follow the guidelines in the person centered plan such as individual safeguards and specific goals determined by the person and their team.
Work site is the persons' home in Sidney and in the surrounding areas in the community. Mileage reimbursement for all service related travel.
Experience working with individuals with developmental disabilities is preferred but not necessary.
Approximately 12-14 hours a week- primarily a day shift with occasional evening/weekend needs. Must have reliable transportation, valid New York State driver's license and a high School diploma or equivalency.
