JOB DESCRIPTION
The Case Management Assistant (CMA) will work under the direction of the RN Case Manager or Social Worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. He/she will coordinate timely referrals and facilitate payor authorizations where applicable. The CMA will also supply support in areas that do not require licensed personnel.
RESPONSIBILITIES
Files department documents, including discharged patient’s files, and maintains files according to established systems.- Assures all forms used by the department are kept in stock
- Organizes and maintains office supplies and material
- Picks up mail from the mailroom and distribute accordingly on a daily basis.
- Photocopies and faxes necessary document as requested by Case Management personnel.
- Accurately types all memos, letters, and reports using Meditech, the PC (Microsoft Word) typewriter; meets timeframes established by department director.
- Maintains Home Health Referral Log and documents all home health referral, DME & D/C facility.
- Contacts all patients discharged with home health services to verify appropriate services are being provided and to assess satisfaction level.
- Records minutes to monthly Case Management staff meetings within two weeks of meeting.
- Documents minutes of monthly Utilization Management meeting within two weeks of meeting.
- Completes other duties as assigned.
- Assists Director with PI Plan and other designated projects.
- Adheres to all regulatory and legal requirements in the performance of job duties.
- High School Diploma or GED required
- 1-3 years of experience
- BLS certification a plus
