Transportation Coordinator 1123R (New Lenox, IL)
Job Title: Transportation Coordinator
Work Location: New Lenox, IL
Work Hours: 8:00am-4:30pm; M-F
Job Description:
- Collaborates with nurses and schedules medical appointments
- Creates weekly transportation schedules, ensuring that all medical appointments, work, and any other activities have transportation
- Completes transportation schedules for day programming
- Works collaboratively with staff to provide transportation in case of staff call-offs.
- Transports clients to appointments if no other staff are available
- Identifies doctors who accept each person’s managed care plan. .
- Coordinates transportation to emergent appointments
- Tracks medical appointment outcomes, ensuring that necessary follow up appointments are scheduled in a timely manner
Qualifications:
- High School Diploma or GED is required.
- Requires strong organizational ability, superior communication skills, and very detail-oriented. Position requires someone who is skilled in meeting deadlines and has excellent time management.
