DUTIES:
- Provides activities on a daily basis that expands and enhances the lifestyle of each resident in the seven categories required by the State and Federal regulations of creativity, physical, emotional, spiritual, group, individual and social.
- Facilitate resident visits with family and friends, following all guidelines associated with outdoor and/or indoor visits including scheduling, infection control protocols, monitoring visit if necessary, etc.
- Develop, plan and lead stimulating activities to residents as determined through resident preference, ability, and interest. Maintains daily documentation for the activity department through resident records, progress notes, one-to-one sheets, participation records and behavior charting. Ensures that all activity department, facility and division policies are maintained. Responsible for daily documentation of participation records, sensory stimulation records, and one-to-one participation sheets.
- Responsible for offering variety of activity choices, maintaining scheduled activities as planned, and facilitating spontaneous activities that meet psycho-social, spiritual, emotional, physical, and creative needs of all residents. Responsible for preparing in advance of the scheduled event, setting up necessary supplies, communicating to residents the activity is about to begin, assisting residents to/from the activity, provides encouragement to residents throughout the activity, maintains positive atmosphere that enhances participation. Seeks guidance for difficult residents, supplies that are not readily available, and any other special needs. Assists with activities of daily living (ADL) care during activities or as needed including but not limited to answering call-lights, and other duties as needed.

- Experience in assisting residents during activity meals.

- Experience working in a long-term care unit.
- Shiftwork required to include, weekends, nights, split shifts and Holidays.
- Must submit to a Standard background check - Name check, CAPS, Drug Screen and Fingerprinting
- Must possess a valid, unrestricted Colorado Driver's License
- Must possess a valid CNA license from the Colorado Department of Regulatory Agencies.
- May be required to transport residents to appointments.
- Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
- On or after October 31, 2021, each facility shall ensure all newly hired employees or support staff members are fully vaccinated at the time of hire or receive their first dose of a two-dose protocol, or a single-dose vaccination, prior to providing any care, treatment, or other services for the facility and/or its clients. Upon hire, new employees will have three (3) business days to provide attestation to their status with proof of vaccination. Vaccinated employees must provide proof of first dose prior to providing care.
- Note: Fully Vaccinated means two (2) weeks after a second dose in a two-dose series of the COVID-19 vaccine, such as the Pfizer, Moderna or Novavax vaccine, or two (2) weeks after the single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine.
