Job description
Summary: Triage nursing staff is responsible for the safe, appropriate and timely evaluation of patient symptoms via the telephone. They are responsible to determine the urgency of care needed, referring to or scheduling appointments with providers, and giving health information and advice to callers.
Goal: The goal of telephone triage services is to improve continuity of care, avoid unnecessary visits to the emergency department, reduce parent and patient anxiety, reduce risks of medical complications and provides information for self-care and symptom management.
Primary Responsibilities:
- Triage phone calls
- Schedule appointments as warranted
- Utilize Barton Schmitt protocols to provide appropriate patient advice and teaching
- Notify previous day sick visit parent/patient for progress update and document feedback
- Notify parent/patient who have recently visited an emergency department or clinic for status and schedule follow-up appointment as warranted.
- Obtain/receive STAT results and forward to ordering provider
- Notify parent/patient of test results (lab values, radiology reports etc.)
- Schedule tests/procedures ordered by provider
- Obtain insurance pre-certification for CT and MRI scans, and schedule accordingly
- Track outstanding lab and radiology results and contact parent/patient for update on status
- Review lab & radiology results when provider in not available, forward abnormal findings to on-call provider
- Contact parents regarding elevated lead results and remedy as provider indicates
- Report results of elevated lead levels of patients residing in New Hampshire to proper state authorities
- Schedule specialty appointments requested by provider
- Verify prescription eligibility within ECW prior to utilizing E-prescribing.
- Manage all refill requests or forward to provider as warranted (controlled substances etc.)
- Monitor Patient Portal for patient inquiries and refill requests
- Obtain newborn records at provider request
- Receive and record discharge summary information from outside sources (inpatient facilities etc.)
- Correspond with Massachusetts Department of Children and Families and document all such correspondence
- Refer parent/patient for crisis evaluation, Sexual Assault Nursing Examiner (SANE) evaluation or ED visit as warranted
Skills/Abilities/Competencies Required:
- Current Massachusetts license to practice as a registered or practical nurse
- Current CPR Certification
- Comply with all local, state and federal privacy and confidentiality rules and regulations
- Good judgement and critical thinking skills
- Ability to follow written and oral instructions
- Compassionate and caring manner with patients and families
- Excellent communication skills; written, verbal, auditory and technical
- Functions well in a high stress environment and under time pressure
- Works well independently
- Ability to prioritize tasks in a busy environment
- Computer skills and software aptitude
Job Type: Full-time
Pay: $30.50 - $37.00 per hour
