POSITION SUMMARY
The Spiritual Care Coordinator is responsible for the provision of patient-directed, spiritual care services for patients, families, and caregivers either directly or through coordination of care with other spiritual care providers.
QUALIFICATIONS
  • Graduate of accredited seminary, school of theology, or a degree in counseling, or equivalent experience.
  • Ability to work in an interfaith setting, with different lifestyles, cultures, beliefs, and values.
  • Knowledge of Homecare philosophy of care.
  • Demonstrates excellent planning, observation and communication skills.
  • Self-directed with the ability to work with little supervision.
  • Experience with patients, families and caregivers dealing with illness.
  • Must have a valid and current PA driver’s license, with reliable, independent means of transportation.
DUTIES AND RESPONSIBILITIES
A. Assesses spiritual needs and provides consultation to patients/families/caregivers or the patient’s clergy if desired in a timely and supportive manner.
B. Assists in spiritual care of patients and family members upon request.
C. Assists patients with referral to their home church or church of their choosing upon request.
D. Assists patients and their families with spiritual care issues which may interfere with obtaining maximum benefits from their medical care.
E. Attends case conferences and team meetings; participates in overall coordination of patient care bringing information regarding psychosocial, pastoral, and physical information affecting the patient and family.
F. Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation.
G. Provides consultation, education, and support to the homecare team on spiritual care.
Documentation
A. Accurately completes required documentation for each visit on the day of the visit and submits it to the office daily, but no less than weekly.
B. Documents all communication and follow-up with coordination of further referrals to patient’s church/synagogue, clergy, social worker, or other appropriate services as needed.
Communication
A. Maintains positive communication with patients/families/caregivers, staff, and other contacts.
B. Maintains on-going liaison and communication with staff.
Professional Standards
A. Maintains a professional standard of conduct, projecting a positive image of the agency at all times.
B. Spiritual care coordinators/volunteers will not be asked to compromise their own beliefs.
C. Complies with risk management practices.
D. Adheres to all policies and practices of the company.
E. Maintains confidentiality of patient and staff information.
Professional Development
A. Seeks clinical collaboration in new and/or unfamiliar care areas.
B. Assures that continuing education credits are met for professional license renewal if appropriate.
C. Participates in staff meetings, care conferences, education training and the QA program as appropriate.
PHYSICAL ACTIVITIES AND REQUIREMENTS NECESSARY TO CARRY OUT POSITION
Twisting/Bending
Twisting and bending is required for movement in and out of the car.
SENSORY ABILITIES
Verbal
Must be able to clearly speak English to communicate, assess, and/or educate consumers, co-workers, referral sources and staff. Must also be able to communicate with physicians, families, other professionals involved in patient care, and other groups of customers.
Auditory
Hearing level or accommodation sufficient to use the telephone and to communicate with staff, consumers, families, physicians, other professionals and other groups of customers.
Visual
Must be able to see to complete/review required documentation and to use the personal computer.
