JOB SUMMARY:
Specialty Care Navigator receives referral inquiries from local, out-of-area, national and international referral sources. Functions as coordinator of care for patients with illnesses or abnormalities. Works closely with internal and external pediatric subspecialists, referring physicians, patient families, community agencies, and others supporting the patient's healthcare needs to ensure comprehensive, appropriate, efficient and timely health care services. Serves as the first line advocate for the patient and family and collaborates with the health care team to anticipate needs, helps to coordinate appointments, assists in implementing effective care plans and educates the family in an appropriate manner. Ensures comprehensive inpatient and outpatient care coordination, assesses additional program needs, assists in the transition to outpatient follow up care and referrals to outside services based on patient needs. Specialty care navigator supports coordination of care for any destination program, patient population or region as prioritized by department leadership and the organization.
MINIMUM QUALIFICATIONS:
High School Diploma, GED or equivalent
Associates Degree and 1 year of experience coordinating care in a healthcare setting; or, if no Associates degree, 5 years of experience coordinating care in a healthcare setting
Competent in utilization of computers, various computer programs for data entry and retrieval, ability to run basic reports in various computer systems
Experience in obtaining services for patients (appointments, durable medical equipment, community resources and others)

PREFERRED QUALIFICATIONS:
3 years of experience
Bilingual English/Spanish

ADDENDUMS

Hawaii and Pacific Rim Services
OTHER DUTIES AND RESPONSIBILITIES
  • Provides information and coordination for families/patients traveling from Hawaii and the Pacific Rim about or seeking services at Rady Children's.
  • Coordinates review of medical records for Hawaiian patients and referring physicians inquiring about RCHSD services.
  • Coordinates healthcare services for patients traveling from Hawaii/Pacific Rim.
  • Provides concierge services (information on lodging, coordination of transportation, translation
services and other resources) as needed.
  • Works collaboratively with other healthcare institutions (Kapiolani, Tripler Army Medical Center, Kaiser Hawaii, Sharp, Scripps, UCSD, etc) to provide services.
  • When requested by RCHSD, travels to Hawaii to provide information and represent RCHD to the hospitals, health plans, physicians and patient families.
International and Out of Area Services, Proton Therapy Services
OTHER DUTIES AND RESPONSIBILITIES
  • Provides information and coordination for families/patients traveling from out of the area inquiring about or seeking services at Rady Children's.
  • Coordinates review of medical records for out of area patients inquiring about RCHSD services.
  • Coordinates healthcare services for patients traveling from out of the area, including international patients.
  • Provides concierge services (information on lodging, coordination of transportation, translation
services and other resources) as needed.
  • Works collaboratively with other healthcare institutions (Sharp, Scripps, UCSD, etc) to provide
international services.
Fetal Program
OTHER DUTIES AND RESPONSIBILITIES
  • Will develop a comprehensive maternal-fetal and post-natal program for the care of patients with complex needs.
  • Works closely with maternal-fetal specialists, community obstetricians, community birthing centers, genetic counselors and others serving the needs of high-risk pregnant women.
  • Coordinates care that meets the needs of high risk pregnant women including behavioral support therapies and services.
  • Prepares the family for their pre-delivery, delivery and post-delivery journey.
  • Coordinates care for fetal program patients delivering at outside hospitals (Sharp, Scripps, Palomar, Rancho and others) including appropriate dissemination of patient health records.
  • Provides concierge services (information on lodging, coordination of transportation, translation
services and other resources) as needed.
  • Ensures fetal patients transition into comprehensive inpatient and outpatient follow-up status postdelivery.
  • Ensures timely communication back to referring physicians on initial diagnosis in addition to
outcomes post-delivery.

The expected hiring range for this position is $25.47 to $35.03
