PRIMARY FUNCTION:

Lead Navigator will act as liaison with project partners; organize and manage all outreach events; carry a navigation caseload; facilitate Seeking Safety and coaching sessions; manage the CRPAs (Certified Recovery Peer Advocates), and ensure linkage and navigation services to clients connecting them internally to Alliance program and/or external referrals to community partners.

DUTIES AND RESPONSIBILITIES:

● Organize and manage all outreach events;

● Carry a caseload of 30 + participants

● Provide linkage and navigation services for care management, mental health, substance abuse treatment, HIV/HEP C Testing, medical care, and PrEP education

● Supervise peer interns: provide 1:1 task supervision, support their development, and follow all guidelines from the “Peer Program” such as implementation of peer development plans.

● Facilitate effective behavioral intervention (VOICES and Seeking Safety)

● Conduct Recovery Coach sessions

● Complete the Global Appraisal of Individual Needs (GAIN-SS) instrument and GPRA tools on program enrollees as often as dictated by grant.

● Facilitate structured socialization events that will reduce isolation; build rapport among staff, peers, and clients; impart HIV/SUD risk reduction information; set the stage for engaging clients in counseling and testing services;

● Prepare for and participant in funder required site visits and audits.

● Regularly attend weekly supervision, program meetings, monthly agency staff meetings, and/or any other meeting at the direction of the agency or supervisor.

● Additional tasks as assigned to meet the needs of the department and/or agency.

● Attend a minimum of two agency events for clients and one agency fundraising event every 12-month period.

QUALIFICATIONS:

The Lead Navigator must have four years’ experience working with a population of low-income, underserved, African-American and/or Latinx men and women, aged 18 and older, who are unstably housed, i.e., living in SRO sites or public and private shelters, etc. and at high risk of HIV infection and/or substance use disorder (SUD). The Navigator should have a High School Diploma or equivalent. The Navigator should have knowledge of community resources and New York State HIV Testing requirements and system. Must be willing to work some evenings and weekends. Bilingual Spanish is a plus.

Job Type: Full-time

Pay: $38,000.00 - $45,000.00 per year
