All Seasons Senior Living is looking for a Resident Care Coordinator at our 52 bed Assisted Living Facility.
This position is for someone who loves to care for others, especially the geriatric population. We are looking for someone who has good people skills, a person who can handle time constraints and think on their feet. Being a Resident Care Coordinator entails all that follows:
Coordinating all resident cares and management of medications under the direction of the nurse and Administrator.
Ordering medications through pharmacy , Hospice Companies, or Express Scripts.
Ensuring monthly bulk medication order is accurate through pharmacy, putting away medications with assistant once order is received
Destroying medications that have been discontinued or expired with assistant
Checking faxes for orders and patient summaries and updating electronic files and Medication Administration Record
Corresponding with doctors and nurses via email, text daily and alerting them to any resident changes.
Communicate with families regarding resident cares, and any changes
Communicating in a timely manner changes with residents to facility nurse
Communicating with residents about care concerns and changes
Building positive relationships with the residents
Sending resident logs via fax per doctor, home health, hospice request
Following up on any direction given from the facility nurse, doctors and or outside services regarding resident care
Collecting ordered samples and ensuring delivery to lab via mail and or provider
Working closely and building positive relationships with doctors who visit the facility
Working with Administrator on service plans
Working closely with Administrator to ensure residents needs are being met and for direction
Working with Administrator on varies to tasks and or assignments to ensure smooth running of the facility
Communicate resident care changes to all staff
Correction and training of staff to ensure resident needs are being met
Required to work shifts on the floor as needed, being open to over time and/or be on call as the care of residents is priority
When doing office days being available daytime business hours to interact with doctors and nurses as they come into facility
Working together with management team to ensure shifts are covered
Learning the different positions within the facility to help out as needed as emergencies arise
Meeting weekly with the Nurse and management team for quality assurance
Must have strong communication skills, ability to stay on task, positive attitude, ability to adapt to change and mindset of residents first
Must have or be willing to get TB test, Cpr & First Aid Certification on or before hire
CNA is preferred but not required; At Least 3+ years of caregiving experience in AL/SNF required
Job Type: Full-time
Pay: $15.00 - $17.00 per hour
