Covina Rehabilitation Center is a looking for a Full-time Registered Nurse to care for our patients and facilitate their speedy recovery. Skilled and Subacute positions available.
*Walk-ins welcomed Monday- Friday 11AM-3PM*
- Note: Candidates must be fully vaccinated against COVID-19 (with Booster vaccine, if eligible) and will undergo rapid Covid testing upon interview arrival.
- Location: Covina, CA (Skilled Nursing and Subacute environment)
- Compensation: We offer competitive salaries that are negotiable depending on previous education and nursing experience
- Duration: Full-Time
- ***SIGN ON BONUS*** (Paid out 3mos. & 6mos. from Start Date and Full-Time applicants only.)
Shifts available:
- 8 hour shifts
- Skilled Unit: Noc (11pm-7am)
- Subacute Unit: Noc (11pm-7am)
SUMMARY: The Registered Nurse is responsible for assuring physicians’ orders are followed and quality care is provided on each shift in a skilled care facility. This position involves 90% direct patient care and 10% administrative non-direct patient care. The goal is to promote the patient’s well-being by providing high quality nursing care. The Registered Nurse assists in providing a clean, safe, dignified, happy and healthy environment for residents by performing the duties as described below:
Essential Duties & Responsibilities include but are not limited to:
- Complies with the facility’s attendance policy requiring regular, reliable, and punctual attendance.
- Makes actual patient rounds, assessing and observing the following at least three times per day:
- Monitoring vital signs as necessary with appropriate interventions as indicated
- Positioning
- Hydration/nutritional status/feeding program.
- Actual patient interviews
- Assessment of new admissions
- Monitoring lab and x-ray values related to patient’s condition o Incontinence and related interventions.
- Adjust and administer patient’s medication and provide treatments according to physician’s orders.
- Skin care and/or changes of skin condition
- Use of proper assistive devices
- Ensures timely response to call lights.
- Acute changes in condition: nausea, diarrhea, infections, LOC changes, confusion, elevated temperature
- Pain management/intervention
- Assess behavior and provide interventions as indicated.
- Wound treatments
- Therapy treatments
- Incidents; falls, etc.
- Patient complaints / concerns
- Provides basic patient care and treatments, such as taking temperatures or blood pressures, dressing wounds, treating skin issues, or maintaining and monitoring catheterizations and colostomies, injections and blood sugar checks.
- Identifies/assesses emergency medical situations and provides appropriate interventions as indicated.
- Documentation and follow-up with timely notification of physician and family
- Care planning as needed.
- Provides a weekly written evaluation in the progress notes by a licensed nurse of the resident’s performance in the bowel and/or bladder management program.
- Performs other duties or functions as assigned by the DON and/or Administrator.
Skills
- Proven experience as a Registered Nurse.
- Previous experience in a SNF or similar job setting would be preferred, but not required.
- Excellent knowledge of nursing care methods and procedures.
- Excellent knowledge of emergency care.
- In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times.
- A team player with excellent communication and interpersonal skills

Required
- Valid and current California RN License
- BLS or CPR (required)
