We are seeking to add multiple detail orientated Registered Nurses (RNs) to join our Hospice and Homecare teams in the following areas:

• Claremont and Surrounding Areas
• New London and Surrounding Areas
• Enfield and Surrounding Areas

A Registered Nurse (RN) Case Manager oversees and coordinates the plan of care for a dedicated case load of patients. This means getting to know your patients, caregivers, and their families while working with a supportive and collaborative interdisciplinary team, focusing on patient-centered care.

Responsibilities for a Hospice or Homecare Registered Nurse (RN) Case Manager:

• Complete home and facility visits during scheduled hours based upon patient and caregiver needs
• Coordinate patient care among the Hospice or Homecare team for an assigned case load in a designated territory
• Maintain accurate, up to date documentation based on current CMS guidelines

Qualifications for a Hospice or Homecare Registered Nurse (RN) Case Manager:

• Proof of up to date Covid-19 vaccination card to present at Orientation or engage in the exemption process prior to hire.Per the CDC, "A person is up to date with their Covid-19 vaccination if they have received all recommended doses in the primary series and one booster when eligible."

• Current NH RN license
• Minimum 1-2 years direct care experience preferred
• Excellent communication skills necessary
