Overview:
Agrace Registered Nurses enjoy having a variety of patients, diagnoses, travelling locally to a variety of settings, such as patient’s homes or health care facilities. As an Agrace Registered Nurse you will collaborate with patients, families, caregivers, physicians, facility partners and Agrace care team.
Responsibilities:
The hours are generally 4pm - 1230am, with an every other weekend rotation. On-call is required monthly between the hours of 12am - 8am, total of 8 hours, choice of employee which date and times. Every other holiday, total of 3 per year.

Begin your shift and end your shift from home. As a registered nurse you will be able to apply your strong clinical skills on a more personal one-on-one level with patients and their families. This position allows you to work independently as well as having team support one call away.

Responsibilities:
As an Agrace Visit Registered Nurse you will be responsible for:
- Working closly with the Patient and Family to create family centered care consistent with the philosophy, goals and standards, of Agrace
- Collaberate with Hospital Staff, Physician, Patient and Family, and Community Partners
- Support with immediate pain and symptom management, education, and teaching
- Nursing assessments, interventions, evaluations, documentation and coordination of care
This position maintains the accountability for the nursing practice and patient outcomes in order to meet our mission of enhancing quality of life for patients and families facing serious illness.

Qualifications:
The ideal candidate will be independent, organized and enjoys a variety of patients and diagnoses. This position travels locally and works in both patient homes and health care facilities.
Agrace prefers candidates to have:
- Associate degree of Nursing (Bachelor’s degree preferred)
- Current, unrestricted Wisconsin RN license
- Three years direct patient-care nursing experience - preferably in ICU, ER, Medical-Surgery, Oncology or Palliative Care or two years relevant health care experience.
- Valid Wisconsin driver license and car insurance and reliable access to an automobile in good operating condition.
