PHYSICAL THERAPIST ASSISTANT- HOME HEALTH

The Physical Therapist Assistant(s) visit Home Health patients for implementation of the physical therapy care plan as established by the Licensed Therapist. Administers prescribed physical therapy treatment programs. Meets the patient's goals and needs and provides quality care. Helps patients accomplish treatment plan and secures necessary supplies and adaptive equipment to facilitate independence in patients.

• Communicate treatment plan and progress with patient, family, agency, and caregivers in conjunction with Licensed Therapist
• Proficient and Experienced in Axxess EMR Software.
• Responsible for planning, coordinating and documenting care for an assigned caseload of patients.
• Collaborate with other team personnel to achieve well-rounded care.
• A great bedside manner and a professional attitude
• Responsible and have an extreme ability to work independently

QUALIFICATIONS

• Current licensure as Physical Therapy Assistant in practicing state and/or nationally certified

• Graduate from a Physical Therapy Assistant program

• Valid CPR card

• Valid Driver’s license

• Valid Car Insurance
