Key Accountability

  • Review physician orders and contact as needed for clarification of order. Dispense correct pharmaceutical in accordance with all laws and regulations.
  • Identify appropriate outcomes of drug therapy.
  • Monitor and document ongoing drug therapy including the assessment of therapeutic appropriateness of the choice of medications(s); therapeutic duplication, appropriateness of dose, frequency, and route of administration; adherence to drug regimen; potential drug, food or diagnostic test interactions of disease limitations to drug use; laboratory and clinical monitoring methods to detect drug effectiveness, side effects, toxicity or adverse effects; and preparing and maintaining clinical records.
  • Consult and collaborate with other disciplines of the health care team to ensure appropriate, safe, and complete therapy is provided.
  • Instructs patient/family/caregivers in administration of the pharmaceuticals.
  • Provides patient, family, and/or caregiver education and information pertinent to pharmaceutical and related safety issues. Use equipment, supplies, and systems safely, effectively, and efficiently.
  • Oversee work of other pharmacy personnel to ensure the dispensing of correct pharmaceuticals per physicians orders.
  • Oversee drug control system including receipt of prescription drugs and orders, storage of medications, packaging of medications, preparation and dispensing of prescriptions, labeling, and packaging of prescriptions for delivery.
  • Perform other duties as requested by Pharmacy Manager to facilitate the smooth and effective operations of the organization.
  • Ensures appropriate documentation is maintained in accordance with regulatory requirements.
  • Reports accurate drug information to physicians, other professional personnel and public in a timely manner.

  • Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines.

  • Complies with all FDA mandates and self-imposed manufacturer restrictions on particular medications, such as REMS programs.
  • Maintain compliance with Personnel policies and procedures.
  • Complete and pass biannual aseptic technique testing as outlined in P & P in a timely manner.
  • Employee is responsible for completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing videotapes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UnityPoint at Home programs and initiatives.

    Qualifications:


    Minimum Requirements

    Education:


    • Bachelor of Science or PharmD. degree.
    • Graduate of an accredited school of pharmacy.


    Experience:


    • One year experience in retail or clinical setting

    • Three years’ experience in retail or clinical setting.


    License(s)/Certification(s):


    • Current license to practice pharmacy in the state(s) where services are provided.
    • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
    • Mandatory Reporter: Child & Dependent Adult Abuse.


    Knowledge/Skills/Abilities:


    • Sufficient knowledge, training, and expertise in pharmacotherapeutics relative to infusion medications as well as specialty medications.
    • Strong interpersonal skills.
    • Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.
    • Knowledge of professional pharmaceutical principles and practices.
    • Ability to work as a collaborative team member.
    • Ability to understand and apply guidelines, policies and procedures
    • Basic knowledge of computer applications (i.e. Word, Excel) and operations.
    • Ability to maintain confidential information
