Proposed Schedule: 20 hours per week. Shifts vary between 8am-4:30pm Monday-Friday.

Note: This position does require some travel to our northern health centers based on the patient needs and will receive mileage reimbursement.

Position Summary

The Peer Counselor will work to assist the Program Manager and the Program Director to successfully meet the objectives of the HCV Program by providing support to patients living with Hepatitis C with engagement and completion of treatment, delivering infection prevention education to those at risk for HCV infection and conducting outreach to identify individuals unaware of their HCV status.

Essential Duties and Responsibilities:

  • The Peer Counselor will provide patient centered support and education to patients living with or at-risk for HCV infection
  • Assist in identifying patients in need of HCV services and treatment including providing rapid HCV testing
  • Assist patients newly diagnosed with HCV to begin treatment
  • Assist patients on HCV treatment to adhere to their medication, attend appointments and complete labs, including escorting/transporting to appointments
  • Conduct outreach to patients lost to or never engaged in HCV care
  • Work in collaboration with the HHHN care management team to assist patients successfully complete curative treatment
  • Conduct outreach and provide HCV prevention, risk, and harm reduction education, testing and linkage to care as needed for patients engaged in MAT (Medically Assisted Treatment) for opioid use disorder at HHHN
  • Maintain records, case notes and activity logs of patient encounters, outreach and testing
  • Abide by all HIPAA and confidentiality requirements
  • Perform additional duties and special projects as assigned

Qualifications:

  • Must be NYS HCV Peer Counselor certified or eligible for certification
  • Ability to effectively and positively interact with ethnically and culturally diverse individuals, persons with active or historical opioid drug use, low- or no-income populations, and individuals with special needs
  • Strong planning, organizational and time management skills
  • Ability to work independently and communicate effectively with team
  • Ability to work collaboratively with treatment providers and care managers
  • High School diploma or higher education
  • BLS Certified or willingness to complete training
  • Familiar with EMR systems and medical terminology
  • MS Office Suite experience
  • Other required training including, but not limited to, motivational interviewing, harm reduction, and conflict resolution
