Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) who's services include providing personalized, affordable, high-quality medical, dental, vision and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their individual needs. Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. East Valley serves the health care needs of uninsured and underserved individuals and families throughout our 8 health center locations.Our mission is to provide access to excellent health care while engaging and empowering our patients, employees, and partners to improve their well-being and the health of our communities.
Position Purpose:
To facilitate the access of homeless and/or at-risk of being homeless individuals and families to medical and social services in a timely manner.

Position Responsibilities and Functions:

  • Identifies patients who need social services (housing, food banks, education support, employment services, linkages to services, etc.)
  • Maintain a calendar of outreach activities, including community events, workshops, appearances, and other communication opportunities.
  • Assesses patients’ needs and develops a case management plan for immediate medical and social needs.
  • Nurture new and old relationships with collaborative partners.
  • Refers clients as necessary to in-house or outside services and coordinates transportation if necessary.
  • Establishes and maintains mutual referral agreements with other social services agencies in the area.
  • Coordinates services with in-house health educator, nutritionist and mental health counselor as necessary.
  • Coordinates in-house services for HIV Care, dental and vision services.
  • Provides in-house trainings in order to foster understanding of homeless/community issues.
  • Works closely with staff at homeless access centers.
  • Participates in homeless shelter coalitions in the local communities and attends meetings as necessary.
  • Coordinates in-services and outreach to homeless shelters to promote EVCHC services.
  • Submits quarterly utilization reports to funding sources as required.
  • Performs quality assurance audits, utilizing required tools.
  • Keeps logs of all encounters by all disciplines.
  • Attends meetings, trainings and in-services as required by the program.
  • Other duties as assigned.

Position Requirements and Qualification:

  • Three (3) years experience with the homeless population/social services field.
  • Preferred experience of 3-5 years within non-profit or outreach activities.
  • Outstanding written communication and presentation skills.
  • Bilingual English/Spanish preferred (read, write, speak)
  • Excellent interpersonal skills
  • Able to work with little supervision
  • Self Starter
  • Good oral and written communication skills.
