Position Summary:

The Patient Services role is essential to provide Access Community Health Centers (Access) patients with comprehensive clinical care and ensure better health outcomes. This position collaborates with other clinical staff to assess patient needs and coordinate services. The main function of the Patient Services role is to provide case management to at-risk patients and/or prenatal patients and coordinate necessary follow-up care.

This position is also responsible for tracking and reporting activities related to case management and referrals.

The position also maximizes the financial benefits to Access and its patients. This position assists patients by pursuing their enrollment in government programs such as Medical Assistance and insurance on the Marketplace through the Affordable Care Act. It also assists under- insured patients by administering Access’s sliding discount program.

Qualifications: Desired candidate may meet a combination of the following qualifications

  • A minimum of a high school diploma or equivalent and health care experience and/or social services experience or associates degree in health care, social services, or related field
  • Basic computer skills to be able to send and receive email and use programs such as Microsoft Excel, Word, and Access
  • Experience in case management
  • Experience in health benefits counseling
  • Experience in women’s health issues and/or working with a prenatal population
  • Experience with diverse cultures
  • Previous experience using Epic medical software
  • Fluent in Spanish and English
