HEALTH CARE COORDINATOR (LPN) Part-time – 20 hours - Flexible

The Missionary Franciscan Sisters of the Immaculate Conception (MFIC) in Newton, MA seeks an LPN to serve as the primary resource for their community concerning the health of the Sisters and to serve as the MFIC clinical liaison and health care advocate. The 54 Sisters live mostly in MA and NJ, with nine in Syracuse, Georgia, and RI. The Health Care Coordinator will ensure adequate health care and personal support for each Sister by:

1. Function as the health care person for each Sister to evaluate her health care needs to determine what level of care is appropriate.

2. Research residences for the Sisters and work with the leadership team to secure appropriate living situations for the Sisters, preferably ones with several levels of care.

3. Communicate with the leadership team as needed, acting as an advisor to both the team and the Sisters.

4. Develop a trusting, respectful relationship with each Sister, demonstrating compassion, professionalism, and patience, and become familiar with the MFIC way of life.

5. Maintain updated medical records in a confidential manner and as prescribed by law and ensure that each Sister’s health care proxy and advanced directives are current. Assist with medical forms and documentation as needed.

6. Ensure that Sisters receive and understand information regarding evaluations, recommendations, and diagnoses.

7. Provide education on various health and wellness topics focused on the needs and interests of the Sisters through newsletters, in person, or group forums.

You must be a Massachusetts licensed LPN (or RN). Long-term care and/or social service experience desired. Ability to work with Microsoft Office. Ability to work a flexible schedule and travel as needed. Virtual contact is also possible at times. MA driver’s license with safe driving record required.
