Position Description

Reporting to the Vice President of Quality Improvement & Regulatory Affairs, the Nurse Consultant will provide consultation to the Iowa Health Care Quality Partners (IHCQP) network related to quality outcome improvement. In consultation with IHQCP staff, the Nurse Consultant will assist with process and system review to determine appropriate actions by the network and its providers to meet quality goals and metrics identified by the IHCQP, with particular attention paid to specific contracted rehospitalization and infection control goals. The Nurse Consultant will work in collaboration with network and provider leaders to formulate action plans and assist with implementation as requested by the client.

Primary Responsibilities

  • Collaborate with IHCA and IHCQP leadership to determine specific needs, metrics and implementation schedules.
  • Maintain schedule of work, both onsite and virtual, which meets client needs.
  • Communicate with client and its network providers to determine visit approach (i.e., virtual access to EHR and remote review of documentation, onsite visit times and frequencies, etc.), provide feedback and consultation via phone and/or email, and provide written reports outlining findings of consultation services.
  • Evaluate network providers’ outcomes as reported by the Centers for Medicare and Medicaid Services (CMS) through the CASPER system, internal monitoring and reporting processes, and other measurement methods as identified by the client and/or consultant.
  • Evaluate individual resident case outcomes through review of medical records, reports from involved parties and other sources of information.
  • Assist network providers in identifying root cause of priority outcome, whether metric is on-track, off-track, met or unmet.
  • Provide regular, comprehensive and objective feedback to client related to outcome improvement.
  • Act as a support system for network providers in their quality improvement and regulatory compliance efforts.
  • Assist network providers in formulation of action plans to improve quality or regulatory compliance outcomes.
  • Monitor and report trends of regulatory noncompliance or behaviors/processes/workflow disturbances contributing to declining quality outcomes.
  • Maintain professional behavior and foster supportive, productive relationships with IHCQP leaders and member partners.
  • Communicate with supervisor and other IHCA staff as needed/requested to maximize impact of work, reduce barriers, and identify opportunities for team member involvement.

Additional Responsibilities

Consult with IHCQP contracted clients to execute two specific project needs.

Implement Rehospitalization and ER Use Reduction Program

  • Identify root cause(s) related to efficiency and quality performance indicators for beneficiaries. Review nursing home and hospital medical records, interview leaders or caregivers, and access/review other sources of information to complete evaluation.
  • Identify network providers who are contributing to the overall outcome either positively or adversely, and develop engagement plans for prioritized providers and their residents.

Create clinical protocols and corrective measures to address identified root cause(s) of measures within contracted goals.
Collaborate with medical professionals or other stakeholders to identify potential solutions to reduce the need for certain beneficiaries to be transferred to the acute setting.
Educate leaders, medical providers, nurses, direct care staff and other members of the health care team related to existing or newly established protocols and corrective measures.
Consult on an individualized basis with providers related to clinical questions and challenges.
Monitor and report progress of individual IHCQP member facilities and aggregate outcome data at a frequency identified by the IHCQP leadership.
Monitor, identify and communicate the recommended changes in the action plan as circumstances insist.
Assist with integration of changed or expanded outcome metrics as the managed care organization (MCO) landscape changes and as identified by the client.

Implement TrainAD ProgramDevelop a comprehensive understanding of the TrainAD program and its recommended integration techniques, modules and timeline.
Identify a meeting schedule with IHCQP and American Health Care Association (AHCA) partners to support communications throughout deployment of the program.
Work collaboratively with IHCQP and AHCA partners to identify a timeline and approach to program integration.
Assist with recruitment of participant member facilities as identified by IHCQP leadership.
Collaborate with medical providers and facility leaders involved in antibiotic prescribing activities.
Provide in-person and virtual training according to program outline.
Provide clinical consultation to providers related to individual situations.
Monitor and report outcomes related to program integration in a format and timeframe identified by the client.
Perform any other requirements as prescribed in the program outline.

Qualifications

  • Position must be filled by a graduate of an accredited school of nursing.
  • Current licensure as a registered nurse (RN) in the State of Iowa without restriction is required.
  • Applicants with geriatric experience or certification are preferred.
  • At least one year of experience in a hospital, long-term care facility or population health management role is required.
  • Experience in which administrative and supervisory ability has been demonstrated is preferred.
