Shift : Monday-Friday, 12:30pm-9pm

Location: AdventHealth GlenOaks

The role you’ll contribute:

Obtains and documents through the electronic medical record (EMR) both timely and accurate medication histories for patients using multiple data sources.

The value you’ll bring to the team:

• Interviews patients, caregivers, or providers to obtain the most accurate medication history based on patient-specific factors (eg , illness, cognition, literacy).
• Accurately uses and communicates with external resources to obtain pertinent information. Documents medication history in the EMR.
• Provides appropriate escalation and appropriate handoffs with complex or incomplete medication histories. Effectively communicates with other members of the health care team.
• Maintains knowledge regarding new medications and dosage forms that may be used in an outpatient/ambulatory setting.
• Monitors multiple patient queues from facilities or EMRs.

Qualifications

The expertise and experiences you’ll need to succeed:

• High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required
• Pharmacy Tech credentialed from the Pharmacy Technician Certification Board (PTCB) preferred.
• Pharmacy Tech credentialed from the National Healthcareer Association preferred.
