Job Summary and Qualifications

We are seeking a Medical Social Worker for our facility to ensure that we continue to provide all patients with high quality, efficient care. • As a Medical Social Worker, you will assess the patient’s psychosocial status, to confirm probably post-acute needs, and to detect any potential barriers to a successful transition in to the community.
• You will collaborate with the case manager, the physician and family to determine the best course of action.
• You will demonstrate knowledge of human growth and development, assessment, range of treatment appropriate to the ages of patients served.
• You will screen all patients on assigned unit/service line by covering the selected number of key screening indicators that consistently signal the need for a more in-depth transition plan evaluation by the social worker.
• You will respond to all consults in a timely and efficient manner.

What qualifications you will need:

• Master's Degree in Social Work required
• 1-3 years experience in an acute care setting
• Basic Cardiac Life Support, or CPR must be obtained within 30 days of employment start date
