The Medical Case Coordinator assures that this mission is the foundation for the delivery of services and carrying out job duties.
POSITION SUMMARY
The Medical Case Coordinator will provide support for the medical team, with core responsibilities divided into 2 main areas: Outpatient Medical Services (OPMS) and general Fora Medical Services.
They will be part of a multidisciplinary team that includes medical providers, nurses, residential and addiction counselors, peer mentors and administrative staff.
The Coordinator will coordinate evidence‐based interventions to Fora patients seeking medication to treat their use disorders and provide general administrative support to the medical department.
The Coordinator should have a kind, non‐judgemental and curious attitude and willingness to learn about harm reduction, trauma‐informed care and motivational interviewing. They must be comfortable coordinating and providing outreach and care transition services to people struggling with substance use and mental health conditions.
ESSENTIAL JOB FUNCTIONS: MEDICAL DEPARTMENT
- Facilitate excellent communication between medical department, other Fora programs and the general public: provide accurate and timely information about medical programs and services, review regular communication from other departments and the public, and forward important information to appropriate medical department staff
- Manage medical department bulletin boards on residential floors: collect content, format and post
- Support medical department management of medications in residential through medication spreadsheet, direct review of patient medication supply and pharmacy documents
- Support clear and consistent communication about medications between medical department and 3.5 level patients
- Manage tracking of injectable medications
- Manage logistics for medical groups in residential services
- Manage logistics of medical services offered to 3.5 level residential patients
- Coordinate follow up on infectious disease screening for residential patients
- Collaborate with other Fora staff to identify opportunities to improve services and implement systems for this improvement
ESSENTIAL JOB FUNCTIONS: OPMS
- Facilitate all steps in patient care for Outpatient Medical Services, including managing appointments, troubleshooting with pharmacies, and supporting referrals to community‐based services, in collaboration with OPMS Hillsboro Coordinator
- Coordinate smooth transitions for patients through Fora services, such as residential to OPMS and Outpatient (OP) or adding OPMS to OP services
- Engage with community partners to create and implement efficient and person‐centered referral pathways
- Support patients in completing essential paperwork such as releases of information
- Facilitate access to harm‐reduction/recovery/treatment services through referral and support of patients connecting with these services
- Attend community meetings and promote program awareness and outreach.
- Follow agency policies and procedures provided by Fora Health, including but not limited to:
Fraud, Waste and Abuse, Conflict of Interest, Boundaries, Ethics, Confidentiality, Drug‐Free Workplace and Harassment Policies
- Attend trainings and meetings as scheduled and required by supervisors.
SUPERVISORY FUNCTIONS
- This position has no supervisory duties.
ADDITIONAL JOB RESPONSIBILITIES
- Special projects and other duties as assigned
- If deemed eligible to drive a company car, do so responsibly, with safety being the first priority and maintain your clean driving record to continue in this capacity.
- Must be comfortable working as part of an inter professional team.
QUALIFICATIONS
EDUCATION AND EXPERIENCE REQUIRED
- Bachelor’s Degree preferred
- 3 years relevant experience in healthcare or behavioral health discipline areas, preferentially with substance use disorder and underserved population experience. Relevant experiences include but are not limited to care coordination, case management, treatment of substance use
disorders, community mental health, chronic illness management, patient education and complex social issues.

OTHER REQUIREMENTS
- Verification of Sobriety (if in recovery), per Oregon Administrative Rules (OAR) standards
- DHS Background Check Approval
- Successful completion of Drug Test upon hire
- Documentation of Tuberculosis test and/or evaluation with negative results or evidence of non‐ communicability
WORKING CONDITIONS
- This position works both in an indoor and outdoor environment with frequent travel between sites or to special events and engaging with community partners at their facilities.
- Position requires some overtime, including evening and weekend work. Adjustment of work schedule may be required.
- Possible exposure to communicable diseases which can encompass a variety of illnesses, viruses and infections, including tuberculosis, Hepatitis A, B, and C, and HIV.
- Possible exposure to upset, angry, severely traumatized, disabled or emotionally disturbed adults, adolescents, and families.
- Exposure to potentially hazardous cleaning chemicals, personal protective equipment provided.
