The client is 88 yr old male. 5'10, 200lbs. He is a fall risk. Needs standby assist while ambulating on his walker, and hand's on assistance when getting up from a sitting position or commode. Needs assistance with bathing, dressing and grooming. At night he urinates in bottles but makes it to the bathroom during the day. Caregivers completely manage medication schedule. Light housekeeping and meal prep. The wife also lives in the home but has her own caregivers. No pets in the home.12 hour shifts 8 pm to 8 am
