LPN

IntegraCare, manager of Senior Living, Independent Living & Memory Care Communities, is currently looking for a service-oriented, dedicated and driven Licensed Practical Nurse (LPN) to support our Wellness department and contribute to making us a “A Great Place to Work and a Great Place to Live”.

Duties & Responsibilities:

· Administers medication and coordinates medical needs of the Residents with physicians, Resident Care Director and family members

· Observes, documents and reports significant changes in Residents' condition, recommending changes in care needs

· Supervises Resident Care team members, determines shift assignments, provides instruction to Resident Care staff and ensures care is provided as instructed

Qualifications:

• An active LPN license in good standing is required
• Active registry status on the Pennsylvania Nurse Aide Registry is a plus
• Previous experience in Senior/Assisted Living is a plus
