Overview:
The Patient Engagement Coordinator assists in ensuring Providence Community Health Centers delivers the highest quality Population Health and Accountable Care. In this capacity, he/she uses culturally and linguistically appropriate methods to assist patients in completing preventative and supportive services, including contacting patients by phone, text, and other means. The Patient Engagement Coordinator submits clinical orders as directed by management under established standing orders and supports general member engagement initiatives as assigned, including developing educational content for social media and website.


Essential Duties & Responsibilities:
  • Works with all members of the Health Care Team, Accountable Care, and Population Health to achieve coordinated, patient-centered care. Demonstrates knowledge of the goals and objectives of PCHC’s Accountable Care and Population Health strategies.
  • Support Population Health by completing centralized patient assessments and screenings through administrative order entry and other tasks.
  • Performs clinical activities under the direction of a supervising medical professional, including clerical work, medication reconciliation, screening questionnaires, chart preparation, entering data into medical records and executing standing orders,telephone outreach, and visit planning.
  • Use tools, such as a Population Health application, to create outreach cohorts, address gaps in care, and generate reports.
  • Provide general clerical support including, but not limited to, filing, answering/rerouting calls, and Electronic Health Record functions according to PCHC Policy & Procedures.

Essential Education, Experience, Skills, Qualifications:
  • Completion of a Licensed Practical Nurse program
  • Valid driver’s license with reliable transportation and proof of minimum auto insurance required.
  • Bilingual Spanish strongly preferred.
  • 2 years of experience in the field of human services/social work required; 3 years preferred.
  • Knowledge of Medicaid and Managed care preferred.
  • Effective communication skills and ability to engage patients to work with Health Care Teams.
  • Ability to work independently and collaboratively. Works well with others and has the ability to motivate peers.
  • Demonstrated cultural competence of community.
Applicants will be required to complete PCHC provided Patient Engagement Coordinator training within 30 days of start date and renewed annually.

Mon-Fri flexible hours between 8am-8pm with an hour lunch or 1/2 lunch
