Responsibilities:
Job Duties/Knowledge/Job Performance:
Initiates, establishes, develops and maintains professional relationships between hospice and various referral sources, e.g., hospitals, skilled nursing, assisted living facilities, RCFEs, etc., physician associations and independent medical clinics/offices in order to assure a consistent base and flow of patient referrals across the life span (perinatal, pediatrics, and adults). Takes a pro-active role in helping to grow the census. Effectively collaborates with admission team to assist in implementation and coordination of services for discharge of patients from hospitals into TEH’s program of care. Educates the medical community about the benefits of hospice services. Displays an ongoing commitment to excellence in customer service to both internal and external customers.
  • Initiates contact with the hospitals and proactively facilitates the hospice referral process while prioritizing patient needs. Maintains good public relations with hospital departments, managing time effectively
  • Effectively collaborates with admission team to assist in implementation and coordination of services for discharge of patients from hospitals into TEH’s program of care, maintains a strong customer service approach.
  • Collaboratively works with the Admissions and Liaison Supervisors to troubleshoot real or perceived problems identified by referral sources and resolve customer complaints.
  • Initiates admission of patients to TEH in coordination with admission procedures while identifying the appropriate level of care. Assists team by doing field admissions as need and situation indicates.
  • Demonstrates competencies with excellent nursing skills and care planning while providing direct patient care.
  • Assesses patient/family need for psychosocial support and implement services (i.e. Chaplain, MSW, volunteer) as indicated
  • Works effectively and collaboratively with The Elizabeth Hospice Managers, Supervisors, Medical Director and community physicians by evaluating and assisting patient/families with the appropriate plan of care.
  • Responsible for the direction of care provided by LVN and HA in accordance with patient’s plan of care
  • Responsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements.
  • Establishes, builds and nurtures relationships with various identified sources in the local and regional medical community including community hospitals. Participates in in-service and marketing activities as needed.
  • Works closely with Regional Liaisons to promote the efforts of The Elizabeth Hospice in the community.
  • Manages end of life symptoms proactively.
  • Participates in training and mentoring of new staff.
  • Keeps current with all department policies and procedures.
  • Other job duties as assigned.
Salary: $37.77 - $56.66 /Hour depending on experience
Work hours: 10 hour shifts 930 a to 8p
Weekend Commitment: 2 shifts
Qualifications:
QUALIFICATIONS:
  • Associate degree in nursing with 1-2 years acute care experience required.
  • Certification as hospice nurse preferred.
  • 1-2 years hospice experience preferred.
  • Current CA Registered Nurse (RN) professional license.
  • Demonstrated ability to work effectively with patients, physicians, community agencies and institutions. Interest and expertise in marketing preferred.
  • Demonstrated ability to communicate effectively with members of the hospice interdisciplinary team.
  • Proficient computer skills required.
  • Demonstrated ability to multi-task and manage crisis situations.
  • Current proof of CPR-BLS and negative TB test result.
  • Current driver’s license with proof of insurance.
  • Bi-lingual (Spanish/English) preferred.
