DEFINITION: Under the general supervision of the Social Services Director, performs coordination and management of the WRPT’s Homemaker Services Program, a program established for elders aged 60 years or older residing on within the reservation boundaries.


ESSENTIAL FUNCTIONS: This list is ILLUSTRATIVE ONLY and is not a comprehensive listing of all functions and tasks performed by incumbents of this class.

TASKS:

Develops, monitors, and manages all aspects of the Homemaker program and requirements. Supervises Homemakers. Compile and assess client data in order to develop service agreements. Monitor and coordinate scheduling of services as established. Referral services as needed to additional services. Other responsibilities include regular checking in of patients, assisting in the development and assessment of services needed, keeping abreast of new or necessary trainings to better serve clients health care developments through training and enrolling in courses. As a Homemaker Services Coordinator, you will also be responsible for complying with Tribal laws related to the protection of elders as well as reporting incidents of alleged abuse.

Assessment and re-assessment of participants for enrollment purposes. Assignment of clients to compatible staff who best meet their needs. Conduct follow-up home visits to clients whenever their needs change. Interface in person or by electronic communication with clients as needed. Conduct annual quality assurance surveys with participants. Participate in meetings or any other training/networking opportunities pertinent to the population being served. Ensure service targets are met on a monthly basis in compliance with contractual terms. Responsible for data inputting into the SAMS system.

Processing of referrals and maintenance of a waiting list. Assist in the hiring of staff as per the Tribe’s policies. Monthly supervision meetings with field staff- organizing, scheduling in- service presentations, and in-house training. Training new hires. Resolving client/staff issues of concern in coordination with Social Services Director. Setting up staff monthly service schedules to ensure consistency in the delivery. Monitor staff performance to ensure the quality of deliverables.

Maintains strict client confidentiality; compiles client and administrative reports; submits daily contact sheet on services provided, etc.; maintains records of observation, progress, activities, and any other pertinent client information utilizing the Homemaker Services Program. Enters and maintains client information into automated data system, reports changes in client’s needs and service agreement.


Provides homemaker services such as light housekeeping, washing dishes, preparing meals and laundry when necessary; arranges transportation for clients needing transportation to pay bills, purchase groceries, etc. makes appointments per clients’ request.


Attends meetings, in-service training, and other related training; provides updates of programmatic issues; assists in emergency situations as needed; participates in community service programs and health fairs to promote awareness of the program; maintains records of all case findings.

KNOWLEDGE, SKILLS AND OTHER CHARACTERISTICS:

Knowledge of various methods of providing homemaker services and referral services for clients. Knowledge of the principles and practices of providing services to elderly and disabled persons. Knowledge of communicable diseases and prevention. Knowledge of available public resources in the community. Skill in interacting with clients. Proficient writing skills and good written and verbal communication. Ability to initiate emergency care techniques in emergency situations without supervision.

PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT: Work involves in-home visits moderate traveling in community to meet with clients.


MINIMUM QUALIFICATIONS:

  • One (1) year of work experience providing elder specific services.
  • Must possess a valid Nevada Driver’s license.
  • Must provide a current DMV driving report.
  • Must be insurable under Tribe’s Insurance Policy
  • A favorable background investigation.

    PREFERRED QUALIFICATIONS:

  • Have a degree in a human service-related field or a combination of education and experience working with the target population.
  • Certified Nursing Assistant Certified
  • College coursework in public health or closely related field
  • First Responder Certified
  • Experience working in a Tribal Community or other Social Service-related program.
