Job Description

The primary responsibility of the Home Care Healthcare Consultant/Director is to facilitate effective communication and smooth transitions of care between referral sources, patients, and families, thereby promoting active involvement in home care. The incumbent works closely with physicians, physician groups, hospital administrators, discharge coordinators, case management staff, and patients/families to educate, provide progress updates, and advocate for the Agency's services. Additionally, they play a vital role in coordinating business activities, managing transitions of care, and facilitating the admission process. The Consultant/Director plans, develops, and coordinates home care referrals for patients while also serving as an educational resource for healthcare facilities and the community, raising awareness about the available home care services. They also organize and deliver educational home care programs and presentations. Someone with a home health care experience

One of the primary duties is to identify and cultivate new accounts and markets within the community, following the guidance of the Director of Business Development and/or Administrator. Additionally, the Consultant/Director offers customer service support to designated referral contacts and ensures that account information remains up to date.

Qualifications

At least 2 years experience in community resources. Knowledge in medical professional community. We need someone who has home health experience.

Job Type: Part-time

Pay: $25.65 - $30.89 per hour
