Proactive Community Services (PCS) is a 501(C) (3) non-profit human service organization located in Flossmoor, Illinois. The geographic service area includes communities in Southern Cook County, Illinois, approximately 30 miles south of Chicago. The mission of PCS is to improve health, social, and economic inequalities for disadvantaged minority adults, families, and youth through programs that empower individuals to take actions to increase the quality and years of healthy life and eliminate health and social disparities.

Position Summary
The HIV/STD Counselor-Independent Contractor provides counseling, testing, and referral services to prioritized high-risk populations of unknown serostatus as prioritized and according to the risk definition classification established by The Illinois HIV Planning Group and the
Illinois Department of Public Health. Testing and screenings are to be done at community settings throughout south suburban communities frequented by the prioritized targeted populations.

The position is responsible for combined units of service for HIV testing and STI urine screening, Gonorrhea and Chlamydia (GC/CT), and Hepatitis C—HCV Rapid Antibody test according to the State of Illinois requirements. Each individual served must meet Illinois HIV Planning Group's prioritized definitions without exception. These populations are Men Who Have Sex With Men, High-Risk Heterosexuals, and Injection Drug Users.

Essential Functions:

• Conduct Risk Assessments for HIV/HCV/STI.
• Conduct HIV/HCV/STI testing and screening.
• Refer confirmatory tests or screenings to PCS HIV Program Manager for follow-up
• Complete documentation paperwork of HIV testing and HCV/STI screening results.
• Identify community testing sites where prioritized populations congregate for testing and screenings.
• Refer clients to culturally appropriate behavioral health interventions.

• Customer Service Skills/Client Focused
• Knowledge of HIV/HCV/STI
• Ability to engage substance use/misuse populations,
• Demonstrated experience engaging and working with LGBTQ
• Proficient computer skills, including data entry
• Good interpersonal, oral, and written communication skills
• Ability to draft behavioral change Action Plans
• Good Time Management skills
• Ability to maintain confidential information in compliance with HIPAA guidelines.
• Ability to Conduct Needs Assessment
• Familiarity with Trauma-Informed Care and Gender-Informed Care

Education and Certification:

• High School Diploma or GED Equivalent.
• Valid Driver’s License required with reliable means of insured transportation
• Mandatory Illinois Department of Public Health Certifications Requirement:

1) Fundamentals of HIV Prevention Counseling and Testing;2) Fundamentals of HIV Counseling TestingPartner Services;3) STD screening training; and4) HIV CTR Forms Training.
Experience:

At least three years of experience or any combination of training and experience would provide the required knowledge and abilities to perform the scope of service. Capacity to conduct street and community outreach targeting the prioritized populations. Experience working with diverse populations. Cultural competence is a must. Must maintain confidentiality regarding agency and constituent information. Must be able to perform flexible hours. This includes after-hours and weekends. Valid Illinois driver’s license with clean MVR; daily access to an insured, operable vehicle. Customer Service Skills/Client Focused.

PHYSICAL DEMANDS: Must occasionally lift and/or move up to 50 pounds.
