Keystone Human Services is currently seeking a Health Services Consultant to join our team of professionals in making a positive difference in the lives of our neighbors and the community.
The Work:
Monitor and coordinate health care services
Assist in developing competencies that will allow individuals to be more involved in all aspects of their health care needs
Educate and train staff on various medical diagnosis, procedures, and protocols as it relates to each individual supported
Collaborate with the interdisciplinary team to promote, maintain and restore health, prevent complications and provide for optimum physical, psychological and spiritual comfort for each individual
Remain abreast of best practices related to health care service delivery for people with intellectual and developmental disabilities
Maintain practices that are consistent with Keystone Human Services policies, procedures, purpose, vision and values
Develop and implement health care services for persons as appropriate according to all federal, state and county regulations and requirements, and in a socially valued manner
Support individuals to become independent, make decisions, and live a meaningful dignified life with valued social roles

Pay starts at $56,000 annually for LPN and $63,000 annually for RN

Minimum Requirements:
- Must possess current and valid RN or LPN licensure in Pennsylvania
- At least three (3) years of work experience in the medical field, preferably in working with individuals diagnosed with intellectual and developmental disabilities
- Ability to apply proficient computer skills, including the use of software and web-based systems
- Must demonstrate effective written and verbal communication skills; ability to read, interpret and apply policies and regulations
- Will have clinical competencies in all aspects of nursing and able to apply nursing knowledge and sound judgement during the nursing process
- Must possess a high degree of independent functioning with minimal direct supervision
- Able to pass all required background checks and clearances
- Must possess a valid PA driver's license and have daily access to a properly insured vehicle
