Position Title: Health Information Assistant

Job-Type: Full-Time

Schedule:

• Monday-Friday, 8-hour shift
• Team rotates on-call weekends once a month (Remote, OT)

Position Summary

The Health Information Assistant provides clerical assistance to the Health Information Department and helps to maintain the medical records of current and discharged residents.

Required Qualifications

• High school diploma or general education degree (GED).
• 1+ year Health Information / Medical Records experience.
• Experience with ICD-10 coding.

Preferred Qualifications:

• 2+ years Health Information / Medical Records experience.
• Previous experience in skilled nursing or long-term care facility.
• Previous Experience with Point Click Care.

Essential Duties:

• Provides coverage at Nursing stations three days a week.
• Maintains electronic health records (EHR) databases.
• Scans all health information into electronic record (Point Click Care).
• Completes ICD-10 coding for new admissions and during-stay diagnosis'.
• Updates reports and other documentation in the appropriate residents’ charts according to established chart order and accepted filing conventions and practices.
• Reminds Physicians for regular return visits to assigned residents according guidelines.
• Conducts audits of admissions and discharges.
• Compiles new admission form packets and maintains adequate supply.
• Assists with creation of admission charts and the disassembly of discharge charts as needed.
• Processes resident admissions orders in coordination with Care Center admissions team.
• Responsible for clinic coordination.
