What does a Habilitation Coordinator do?
A Habilitation Coordinator plays a key role in working with the people we support and their staff, to create an individualized support plan by assessing the person's interest and skills along with their needs. The Habilitation Coordinator will monitor the progress of each plan and ensure plans are being implemented properly.
Qualifications:
- Bachelor's Degree in Human Services or related field plus at least one year experience working with people with developmental disabilities is required
- OR Associate's Degree in Human Services or related field plus four years' experience working with people with developmental disabilities is required
- Valid NYS Driver's License with 2 years of experience required
