Job description
The Regional Patient Educator is primarily responsible for facilitating and performing 1:1 (and sometimes group-based) Home Modalities education to in-center patients, with a special focus on patients within their first 90 days of dialysis (i.e. incident patients). Other responsibilities include mentoring, teaching, supporting, and working closely with the Home Ambassadors from each of the in-center facilities in the region, as well as providing meaningful and regular Home Modalities education to teammates in the region, by coordinating facility Peritoneal Dialysis (PD) Lunch & Learns and supporting educational segments in Home Rooms. The Regional Patient Educator is also responsible for facilitating communication and follow up between in-center and Home clinics, ensuring that patients have a timely and smooth transition to Home.

Meets with all potential candidates, including prevalent patients, for Home dialysis to provide necessary education/materials and help identify and overcome barriers. Meets with all new starts within 3 weeks of patients’ first day of dialysis
Engages with patients and teammates using Right Thinking, exemplifying a high level of integrity and compliance, empowering patients through superior education and honoring their choice
Models DaVita mission and values to all new and current Home Ambassadors
Maintains current clinical knowledge of Home Modalities and ensures that education needs are met in accordance with DaVita’s policies, procedures, and guidelines
May participate in the development and ongoing revision of educational materials
Participates in clinic lobby days (i.e. patient-focused), lunch and learns (i.e. teammate-focused), and home rooms as directed by the Home Program Manager
Provides support to Home Ambassadors in basic reporting and documentation skills, Match-D utilization, knowledge/communication development, and accountability measures
May participate with FA in helping select facility Home Ambassadors and in providing appropriate training to encourage Home Ambassador retention
Reports Key Performance Indicators to HPM bi-weekly, including # of clinics visited, # of patients educated, # of patients who selected a home modality, and # of patients converted to Home in that time period.
Tracks progress of patients in transition from ICHD to a Home modality in detail, partnering with Home teams to be sure all necessary steps have been taken without delay

Salary/ Wage Range
$56,500.00 - $83,000.00 / yearCompensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
