The Correctional Facility Program Specialist is responsible for coordinating all activities related to the implementation, maintenance, and integrity of all initiatives stipulated in the correctional facility contract(s). They will directly support the Health Services Director or designee to successfully implement and maintain these services in accordance with OASAS regulation and agency policy and procedure.

Hours: Monday - Friday, travel and attendance possible for offsite meetings

Essential Job Functions:

  • Maintain retrievable records on the utilization of services
  • Ensure each medical record meets Credo P&P and the terms of the specific contract. Coordinate with the correctional facilities regularly to ensure compliance. Coordinate with incarcerated individuals before and after release to offer and track continuation of services.
  • Collect, review and coordinate required paperwork from DOCCS facilities, county correctional facilities and Credo
  • Coordinate logistics of pick up and drop of medications and documentation according to the contracts
  • Follow Credo policy and procedure
  • Coordinate meetings between entities, liaison, single point of contact for the correctional facilities
  • Gather information needed to update electronic health records to assist with referrals and present to Health Services Director for next step.
  • Scanning, faxing, emailing between contacts to coordinate the contracts and services
  • Communication with all entities, maintain positive relationship between everyone
  • Manage referrals, schedule necessary appointments
  • Support and assist the Director and designees with relevant program responsibilities related to program oversight, trainings and meetings.
  • Explain Credo services and treatment system to correctional facilities
  • Discharge coordination between internal stakeholders to ensure proper level of care upon discharge
  • Escalate business issues that affect our ability to complete the process within Credo
  • Maintain clinical documentation upholding all 42 CFR/HIPAA regulations and confidentiality protocols to record each interaction with every patient served
  • Attend staff meetings, in-service and conference/trainings and scheduled supervision sessions as assigned
  • Must maintain a positive and respectful attitude, be able to maintain sharp focus, be extremely organized and do what it takes to assure objectives are met on time
  • Convey information inter/intra agency; communicates with team regarding changes and conflict.
Requirements:

Minimum Job Qualifications:

High School Diploma/GED

And

Minimum of 2 years project management experience that includes implementation, maintenance, analysis and coordination of initiatives while working collaboratively with various stakeholders including professionals/para-professionals, clients or patients, family members, agency and community leaders, as well as state representatives (experience within the harm reduction or substance use field is preferrable).

Or

Equivalent combination of training, education, and experience that would provide the required knowledge and abilities would be considered.

And

Experience and ability to provide written and verbal reports, presentations and other communications that take into account cultural competency and health literacy at all levels, while maintaining appropriate confidentiality when applicable.

And

Must subscribe to a Professional Canon of Ethics.

And

Demonstrated ability to provide friendly, courteous and helpful service

And

Support a Person Centered Approach to Care

Must maintain current certification and/or training as specified by state law and agency regulation. Must have a valid, unrestricted driver's license
