Position: Continuity of Services Coordinator [0082-3] E

Program: Amarillo, Texas – Continuity of Care

Salary: $46,009.60 Annually

Shift Available: Full Time

Essential Duties and Responsibilities:

  • Staff will maintain ethical and professional communications at all times using respectful Trauma Informed Care language
  • Performs enrollment activities for the Home and Community-based Services (HCS) program; completes Permanency Plans, as needed, on designated individuals; and searches for residential placements as referred by Service Coordination, and as per request of a guardian
  • Assists with placement activities for individuals being referred to the State Supported Living Centers
  • Adheres to the TPC Business Code of Conduct and reports concerns of ethical and/or business standards defined in the code that are compromised
  • Will work under the leadership of the Coordinator of Continuity of Services

Minimum qualifications to be successful in job:

  • Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, medicine or other related human services field
  • At least two (2) years experience of case management
  • At least two (2) years of experience working with persons with intellectual and developmental disabilities
  • Graduate work in a related field may be substituted for up to one (1) year of experience
  • Computer skills, including Microsoft Work and Outlook
  • Must have a valid Texas driver’s license and be insurable under agency vehicle policy
  • Must be 18 years or age or older

Preferred qualifications to be successful in the job:

  • The ability to organize documents for assigned caseload and meet time deadlines
  • Ability to communicate effectively verbally and in writing
