Salary Range: $55,265.60 - $64,355.20

Job Summary: This position is focused on providing full-time housing navigation services to individuals and families experiencing housing insecurity in Milwaukee County. This position will work to secure indoor placements in the community for consumers referred by healthcare partners. Services will include benefit establishment, linkage to mental health/substance abuse treatment, negotiating with landlords, and assisting consumers in finding and maintaining permanent housing

Job Duties:

  • Housing navigation services. Collaborate with health care partners, Impact 211, and case management organizations to ensure successful housing placements.
  • Coordinate care. Maintains linkage with Impact 211 mobile health screener to find successful housing placements.
  • Benefits acquisition. Assist individuals with gaining income and insurance
  • Additional units. Work with existing and potential landlords to find additional units for the program.
  • Milwaukee Continuum of Care. Attend monthly meetings for the Milwaukee Continuum of Care and participate in sub-committees.
  • Perform. Other duties as assigned.

Minimum Requirements:

  • This position requires you to have completed the vaccination against COVID-19 and provide proof of a completed vaccination as a condition of employment or submit a completed medical or religious exemption and accommodation request form
  • Current resident of the State of Wisconsin.
  • Current Wisconsin Driver's License and access to a reliable insured vehicle; both maintained throughout employment.
  • A Bachelor’s degree or higher in Social Work or a Human Services related field from an accredited college or university.
  • At least one (1) year of experience working with the homelessness population, housing, social service and/or human services field.
  • At least Intermediate knowledge of all computer and software applications such as Microsoft Word and Microsoft Excel.
  • Must be able to travel extensively, up to 70% of work time, within the community

Knowledge, Skills and Abilities: Knowledge of housing programs and HUD regulations as they relate to homelessness and income guidelines; knowledge of available housing resources; knowledge of the permanent supportive housing model; ability to conduct clinical assessments to provide for proper discharge planning; ability to work with individuals with a wide range of needs.
