Position Purpose: The Community Health Worker (CHW) will work on the community van as part of a team dedicated to increasing awareness and access to colon cancer screenings and health education among underserved populations. The CHW will be responsible for building relationships with community members, providing culturally appropriate health education, and connecting individuals with necessary resources and services. They will collaborate closely with the Licensed Practical Nurse, driver, and other team members to ensure the project's success.

  • Build and maintain relationships with community members, local organizations, and healthcare providers.
  • Provide culturally appropriate health education on colon cancer, screenings, and other relevant health issues.
  • Assist with the distribution and instruction of Cologuard kits to eligible individuals.
  • Facilitate communication with non-English speaking patients using interpreter services.
  • Collaborate with the Licensed Practical Nurse and driver to ensure smooth van operations.
  • Assist in the identification of community locations and events for van outreach efforts.
  • Maintain patient records and ensure confidentiality of patient information.
  • Provide support and resources for patients requiring follow-up care or referrals.
  • Assist in the evaluation of the project's success, including tracking patient satisfaction and outcomes.
  • Advocate for community members and connect them with appropriate resources and services.
  • Participate in team meetings and continuous professional development activities.
  • Maintain a clean, safe, and welcoming environment within the community van.

Education:

  • High School diploma or equivalent
  • Completion of a Community Health Worker training program or certificate preferred

Years of Experience:

  • Minimum of 2 year related in community health, public health or related field

Specialized Knowledge:

  • Knowledge of colon cancer screening procedures, specifically the use of Cologuard kits.
  • Understanding of culturally competent care and experience working with diverse population
  • Familiarity with relevant health issues affecting the target population.
  • Strong communication, interpersonal, and relationship-building skills.
  • Ability to work effectively in a team environment and adapt to changing situations.
