FULL-TIME COMMUNITY BASED CASE MANAGER - HAMILTON COUNTY, OH
Family Resource Specialist
JusticeWorks is looking for a Full Time Community Based Case Manager immediately in Hamilton County, OH. The qualified Community Based Case Manager will assist at-risk youth and their families in accessing and navigating community resources.
To provide excellent service, our families are our priority so working with their schedules is key to creating a healthy rapport with them. Evenings and/or weekends may be required to fulfill a 40-hour work week.

The qualified Community Based Case Manager will have:
- Bachelor's degree or high school diploma and experience working with children, youth, and/or families in a variety of settings or environments.
- Valid driver’s license, auto insurance and access to a vehicle.
- Pass current, applicable clearances.
- Pass pre-employment, post-offer drug screening.
- Ability to work non-traditional hours including evenings and weekends.

The qualified Community Based Case Manager will:
- Assess and engage families in service planning based on demonstrated needs.
- Plan and implement appropriate interventions using evidence based and best practice approaches.
- Maintain open communication with referring authorities.
- Attend court hearings and provide testimony as required.
- Submit accurate, required notes within 24 hours of service and complete reports within assigned timeframes.
- Transport clients utilizing personal vehicle, as necessary.
