Position Purpose: Under the direction of the Regional Director, Pharmacy and Wellness, the Ambulatory Clinical Pharmacist is responsible for providing clinical pharmacy care to Steward members and clinical pharmacy support to Steward Health Care Network (SHCN) clinicians. This position supports Steward risk members with chronic conditions and medication-related issues. This position also supports the network providers' efforts at meeting risk contract quality incentives related to pharmacy and chronic disease measures. The Ambulatory Clinical Pharmacist will implement programs aimed at improving patient care, meeting quality measures related to pharmacy and chronic diseases, and decreasing pharmacy costs for patients and the network. This position is involved in clinical, operational, and health plan components as necessary to support the network. This position is remote based.
- Identifies and telephonically manages assigned patients with complex medication-related issues and coordinates care with care management teams and clinicians.
- Partners with care management staff and SHCN clinicians on matters pertaining to pharmacy and medication management.
- Conducts provider-specific academic detailing on pharmacy utilization and opportunities for practice improvement.
- Provides drug information services to patients, care management staff, and SHCN clinicians.
- Supports clinical pharmacy and SHCN risk and grant initiatives related to medications and clinical care.
- Works with other Steward teams to drive quality measures with a clinical/pharmacy focus.
- Communicates relevant pharmacy updates to SHCN clinicians on a timely basis.
- Develops and provides presentations to clinicians and non-clinical staff as part of the overall clinical pharmacy and performance improvement plans.
- Provides strong customer service orientation including oral and written communications in a manner appropriate to the audience (e.g., patients, clinical staff, non-clinical staff).
- Provides clinical care in a manner that demonstrates and supports a culture of diversity, equity, and inclusion.
- Functions within a matrix environment.
- Performs other functions as assigned.
Education:
- PharmD from an ACPE-accredited school of pharmacy with PGY1 residency experience in ambulatory care, primary care, or managed care, or equivalent experience in ambulatory care, primary care, or managed care.
Years of Experience:
- Minimum of 2 years of telephonic or direct patient care experience with a health care provider or in a health plan setting.
Specialized Knowledge:
- Active, unrestricted license as a registered pharmacist (RPh) in Massachusetts.
- Experience in interacting with an interdisciplinary team including clinicians and non-clinical staff, in educating clinicians, and in using evidence-based medicine.
- Experience in managing patient populations or drug use management.
- Superb verbal and written communication skills.
- Medical literature evaluation skills.
- Understanding of evidence-based medicine.
- Excellent customer service skills.
- Excellent current knowledge of managed care environment.
- Ability to work effectively both in a team environment and independently.
- Ability to prioritize workload effectively.
