Job Summary
The Clinical Documentation Integrity Liaison is responsible for working with senior leaders to improve the overall quality and completeness of clinical documentation across the health system. Incumbents in this position will use a variety of information and reports to guide leaders to engage their team to improve clinical documentation. This position functions as the interface between facility based CDI Specialists and assigned leaders. As the primary CDI contact, the CDI Liaison is responsible for management of provider relationship and education. Job Description

MINIMUM QUALIFICATIONS

Education: Bachelor's Degree in Nursing required; MSN preferred

Licenses/Certifications: Current RN license in Texas or compact state; Certified Clinical Documentation Specialist (CCDS); AHIMA ICD-10-CM/PCS Trainer preferred; Certified Coding Specialist (CCS) preferred

Experience / Knowledge / Skills:
  • Three (3) years of CDI recent experience
  • CDI auditing or education experience; coding experience; previous CDI experience in an academic institution, preferred
  • Ability to formulate clinically compliant and credible physician queries.
  • Advanced knowledge of disease processes and current treatment modalities; working knowledge of official coding guidelines.
  • Advanced knowledge of CMS star rating, Vizient, and US News and World Report rating systems and the ability to use this information to design and communicate clinical documentation improvement opportunities.
  • Ability to work independently, self-motivate, and adapt to the changing healthcare arena.
  • Proficiency in computer use, including database and spreadsheet analysis, presentation programs, word processing, and internet research.
  • Working knowledge of federal, state, and private payer regulations as well as applicable organizational policies and procedures.
  • Working knowledge of quality improvement theory and practice, core measures, safety, and other required reporting programs.
  • Meets performance standards by setting goals and objectives, prioritizing work, and using available resources efficiently and effectively.
  • Excellent verbal and written communication skills, analytical thinking, and problem solving with strong attention to detail.
  • Proficiency in organizational skills and planning, with an ability to multitask in a fast-paced environment.
  • Sufficient fluency and proficiency in speaking, reading, and writing the English language necessary to perform the essential functions of this job, especially with regard to activities impacting patient or employee safety or security.

PRINCIPAL ACCOUNTABILITIES
  • Provides strategic, ongoing education to physicians, mid-level providers, allied health practitioners, nursing and care management regarding documentation, queries, coding guidelines, clinical terminology and coding updates to improve their understanding of severity, acuity, risk of mortality, and DRG assignment.
  • Facilitates modifications to clinical documentation through extensive interaction with physicians, mid-level providers, nursing staff, other patient care givers and coding staff to ensure that appropriate reimbursement is received for the level of service rendered to all patients
  • Effectively and appropriately communicates with physicians and other healthcare providers as necessary to ensure appropriate, accurate and complete clinical documentation that reflects the course of treatment provided to the patient. Escalates organizational queries per system guidelines.
  • Participates in service line meetings, including feedback on outstanding issues, presentations for educational opportunities and any other needs identified by the CDI group.
  • Tracks and trends metrics to provide education and feedback to providers on areas of concurrent documentation improvement. Gathers and analyzes information pertinent to documentation findings and outcomes and uses this information to develop action plans for process improvements.
  • Collaborates with ISD/Informatics teams to update provider documentation templates or tools.
  • Performs and summarizes findings from ad hoc project reviews assigned for various purposes and goals.
  • Remains informed of the latest developments, advancements, and trends in the field of documentation improvement and coding by attending related internal and external seminars/workshops, accessing information from ACDIS, professional journals/Coding Clinic and maintaining certification or licensure. Integrates knowledge gained into current work practices.
  • Works on-site as needed in the hospital as a physician resource, developing physician and medical staff relationships. Acts as on-call provider resource for questions and education.
  • Collaborates with the CDI, Coding and Quality departments and providers to identify and resolve documentation patterns and discrepancies.
  • Maintains CDI department database and spreadsheets on medical staff education regarding documentation, queries, coding guidelines, medical terminology and coding updates.
  • Identifies patterns, trends, variances, and opportunities to improve documentation review processes.
